Davidson HSIRB Protocol Application: Exempt or Non-Exempt (06/2022)
This application reflects changes to our application form effective June 29, 2022.
For projects that meet the definition of research, collection of data may not begin until you have received an approval for your Exempt or Non-Exempt protocol. You may use this form to submit your protocol.
STUDENT PROJECTS - Special Instructions
Student applicants must invite their faculty sponsor/s to collaborate on the application so the faculty sponsor/s may receive reviewer comments and updates. Click the Invite Collaborators link at the top right of this box, under the social and follow buttons. Your faculty sponsor must accept this invitation BEFORE you submit your protocol.
Faculty serving as faculty sponsor for ALL student research projects (exempt and non-exempt) are required to complete human subjects research training before student human subjects research can begin. Faculty must attach their current (un-expired) certification to their faculty sponsor forms for both original applications and modification requests. Please check the date on your certificate before uploading.
HUMAN SUBJECTS TRAINING FOR INVESTIGATORS
All investigators on non-exempt research projects are required to complete human subjects research training before human subjects research can begin. You must attach the current (un-expired) certification for ALL investigators with this protocol application. Please see the Training page for more information.
Human subjects research training is recommended for exempt research but not mandatory at this time. If, however, a request for exemption is deemed to be non-exempt, training must be completed prior to approval.
